Introducing the LiveWorld AI Brand Reputation Index: How Does Your Brand Show Up in AI?

AI is becoming the first-place patients go for information. The question is: when someone asks AI about your product, what kind of story does it tell?

That’s why we created the LiveWorld AI Brand Reputation Index—our inaugural study that ranks 50 of the world’s top-selling pharmaceutical brands based on how they’re perceived by AI platforms like ChatGPT and others. This isn’t just about search rankings or ad visibility. It’s about what AI models are synthesizing from clinical data, HCP sentiment, patient reviews, and online discourse.

Why AI Brand Reputation Matters More Than Ever

More than 30% of patients are already using AI tools for health-related questions—and that number is rising fast. At the same time, HCPs are using AI-powered platforms for decision support, quick research, and product comparisons.

But here’s the thing: AI doesn’t operate like a search engine. It doesn’t simply show results based on ad spend or SEO strategy. Instead, it blends structured clinical data with unstructured public sentiment—things like Reddit posts, patient testimonials, media narratives, and HCP recommendations—to form a composite picture of your brand.

It’s becoming more and more important to know how your brand stands up in this new environment.

What We Found

We analyzed 50 of the leading  pharmaceutical brands using a proprietary framework of seven AI-driven reputation factors, including efficacy, safety, HCP trust, affordability, and digital presence. Here’s what stood out:

Market Leaders Have the Right Mix

Brands like Keytruda, Dupixent, and Opdivo are winning in AI perception. They combine strong clinical performance with robust marketing support, earning trust from HCPs and patients alike. Their digital presence reinforces their standing in AI-generated content.

Trikafta/Kaftrio Stands Out—Without Big Ad Spend

Trikafta/Kaftrio landed near the top of the rankings not because of flashy marketing, but because of transformative clinical results and organic patient advocacy. It’s a powerful example of how real-world performance can drive AI reputation.

Weight-Loss Buzz Drives Consumer-Led Winners

Mounjaro and Ozempic benefit from massive consumer attention, thanks to the GLP-1 weight loss boom. Even their sister brands (Zepbound and Wegovy) are shaping public narratives. AI models are clearly picking up on this surge in media coverage and consumer discussion.

Some Brands Are Struggling to Be Seen

Rybrevant and Bavencio, despite clinical potential, haven’t broken through in the AI landscape. With limited digital presence and patient engagement, AI models don’t rank them as highly, pointing to a need for stronger HCP advocacy and content strategies.

Safety and Cost Concerns Leave a Mark

Brands like Xeljanz and Revlimid are penalized in AI rankings due to historical safety concerns and pricing perceptions. Even when issues have been resolved or mitigated, AI models continue to reflect older negative sentiment.

What Brand Managers Should Do Now

This is no longer just a “nice to know”—your AI brand reputation is shaping perceptions, influencing decisions, and impacting trust. Here’s what we recommend:

  • Monitor your AI reputation across multiple platforms.
  • Optimize your structured and unstructured content to be AI-friendly.
  • Engage HCPs and patients to build positive digital narratives.
  • Address pricing, safety, and misinformation proactively.
